  2.                     TRIP PERMISSION SLIP

  4. Rebecca Tharpe, referred to as PARENT, is the parent and lawful
  5. guardian of Jason Tharpe, a minor, and agrees: 
  6.  
  7. Orange District School Board is organizing a trip for the purpose
  8. of: 
  9.  
  10. visit Big City planetarium 
  11.  
  12. on April 7, 1995 through April 8, 1995. 
  13.  
  14. Jason Tharpe has the permission of PARENT to participate in this
  15. trip and  all activities thereof. Edna Crowell, Martha Knowles and
  16. the other employees and  adult agents of Orange District School
  17. Board are herewith given the following authority  on the dates
  18. stated above: 
  19.  
  20. To consent to any medical treatment that may be required by Jason
  21. Tharpe  in the place and with the same authority as Rebecca
  22. Tharpe.  
  23.  
  24. Further, in consideration of the services performed by Orange
  25. District School Board and  the employees, servants and agents of
  26. Orange District School Board, Edna Crowell, Martha Knowles and the
  27.  other employees and agents of Orange District School Board are
  28. herewith released from  liability for all actions taken in good
  29. faith during the trip.
